Fidelity and Dosage of the Tai Chi for Arthritis and Fall Prevention Program
From Dr Lam: I have been a family physician for more than 40 years; I have prescribed numerous medications. The dosage of medication is quite different from tai chi dosage. For many organizations the right tai chi dosage means what participants need to do to achieve certain health benefits, based on medical studies. For example, how many lessons do you need to attend before your rate of fall be reduced significantly?
Government bodies around the world have funded tai chi to improve health and prevent falls. They need to know the outcome. So, the “right tai chi” being taught at the “right dosage” is important. Therefore, it is important for us to be clear about the dosage. I believe doing any tai chi program for any length of time will bring health benefits, but of course the more tai chi the more health benefits. Very importantly to know the dosage is to say that studies have proven doing this much of tai chi lessons will improve these aspects of health, like how much medication would help certain condition/s.
The Tai Chi Arthritis and Fall Prevention is recognised by many governmental bodies around the world. The USA Administration for Community Living classifies this program as a Title III-D evidence-based falls prevention program. It is essential all our instructors, senior trainers and master trainers have fully understood the dosage for fall prevention. It is easy to remember.
The dosage is what participants must do to gain the benefits of less falls.
Participants must:
• Attend a minimum of 16 hours of tai chi lessons either in person or virtual. This can be accomplished by attending one hour per week for 16 weeks or two hours per week for eight weeks. Participants must attend at least one class per week and be encouraged to practice at home. They should report about home practice at the next class. Most grants allow two to four extra sessions just to be sure participants will gain substantial health benefits even if they missed a few lessons.
• The class must be conducted by current TCHI Board certified instructors at:
• Instructors should strongly encourage participants to practice the tai chi program at home for half an hour daily, at least four days per week. This can be done in one half hour session or two fifteen-minute ones. Naturally, practice within the participant’s comfort zone is important.
o An instructional DVD or online lessons is available to help guide learning and home practice as well as other educational aids such as books, handbook and wall charts.

Tai Chi for Arthritis and Falls Prevention is based on the traditional Sun style of Tai Chi and is easy to learn, effective and safe. It improves flexibility, muscle strength and balance and has been recommended as an effective falls prevention activity by the US Centers for Disease Control & Prevention. The Tai Chi for Arthritis program is designed by Dr Paul Lam in conjunction with a team of medical specialists and Tai Chi experts and scientific studies have proved its effectiveness in assisting pain relief, quality of life, balance and falls prevention. Not only does medical research confirm these benefits, but many Tai Chi practitioners affirm these benefits through personal experience.
Tai Chi For Health, Tai Chi For Arthritis for Fall Prevention Seated 42 (TCA 1 & 2).
Seated modification of the TCA 1 & 2. This a guided instruction through the Seated form of the Tai Chi for Arthritis for Fall Prevention. The form includes the Basic 6, Extension, and the TCA 2 sequel. The form is based on Sun Lu Tang's Tai Chi form which has been adjusted to address the needs of people dealing with Arthritis and Fall Pevention. It also provides a nice introduction for those who have never tried Tai Chi or other internal Martial Arts.
Whille the seated form obviously addresses those who cannot comfortably do a standing form, it also provides an opportunity to study and develop the internal principles, skills and abilities of Internal Chinese Martial Arts. Qi, Song (Soong), I (ee), spiral energy, 5 elements and medical qigong are some examples of what can be enhanced when doing the seated form. The seated form bridges still seated meditation with moving meditation, focusing on the health applications vs the fighting/self defense applications that exist in the movments. When practicing in this mindset, the form is Qigong.

I am going to talk about tai chi, the Tai Chi for arthritis program, how it works, the evidence and how to utilise Tai Chi for Arthritis for health, pain relief and fall prevention... and to gain many health benefits.
Here are 35 published studies on the Tai Chi for Health programs. You can see them online at Tai Chi for Health Institute website. I will highlight three studies here.
1. A study comparing Tai Chi and Hydrotherapy, and control
2. The Sydney Tai Chi Trial.
This is one of the key pieces of evidence why the Centers of Disease Control and Prevention (CDC.gov) and the National Council on Ageing (NCOA.org) in the USA, and health departments and Arthritis Foundations around the world recommend Tai Chi for Arthritis for health and fall prevention.
So how does tai chi work? How would this seemingly soft and gentle exercise prevent falls, relieve arthritis pain, improve muscle strength, improve balance and bring about a better quality of life?
There are three main reasons:
First, it is an exercise involving the whole body, all muscles, ligaments, bones, organs and the mind. The exercise is focused on physical and mental balance - an integral part of tai chi essential principles is balance.
Secondly, tai chi exercise the mind, and integrates mind and body. Falls have a close connection to the mind. When people have fear of falling they are more likely to fall. Tai chi is an exercise to help people to be more mindful, serene and happy. This contributes to reducing falls and relieving pain and the symptoms of arthritis. In fact the mind influences everything, a positive mindset improves healing ability, quality of life and relieves pain and improves diabetes control, just to name a few.
The third one is a concept of qi. Tai chi is designed to cultivate your qi most effectively. According to Traditional Chinese Medicine qi is the life energy that circulates through the body to maintain health and wellness. Disturbance to the flow or the quality of the qi leads to various diseases.
With or without the qi concept, holistic medicine, that the body and mind work as one makes good sense. Tai chi helps people by exerting its healing power on the person as a whole, for example, arthritis does not affect only the joints, but the entire body including the mind. This leads to depression and anxiety, which in turn makes the condition worse. When the person is healthier mentally, he or she will heal better and experience less pain. That is why tai chi works so extraordinarily well.
The third study illustrates how effective Tai Chi for Arthritis is and how quickly the health benefits came. It is by Dr Lee Callahan and colleagues from the University of North Carolina.
Nowadays it is widely accepted by the scientific, orthodox and medical world that tai chi is effective for many aspects of health.
